  • This position is integral in participants' care through the assessment of needs, development and monitoring of treatment plans and progress, and facilitating interdisciplinary approaches to care.

    • Salary: $17.00-$18.00 per hour; up to 40 hours per week; nights and weekends may be required based on organizational activities and scheduling

    • Location: Albuquerque, New Mexico

      • Reports to: Co-Directors

    • Conducts participant orientations with new participants or current participants who need additional assistance accessing resources. 

    • Determines participants' needs by completing intake interviews; determines need for medical, psycho-social, legal, and emergency resources.

    • Establishes an access plan for participants taking into consideration their life circumstances and barriers to accessing care, programs, and services by arranging resources, including transportation and escort if needed. 

    • Monitors participants by ongoing interactions with participants during service hours. Advocates for needed services and entitlements; obtains additional resources; intervenes in crises; providing personal support.

    • Integral in assisting with the creation, evaluation and updating system used to track participants records and interactions.

    • Communicates participants' progress by conducting meetings on a regular basis and disseminates results and obstacles to the support team.

    • Improves participant results by studying, evaluating, and redesigning processes; implementing changes; rewrites policies and procedures, as needed.

    • Meets budget by monitoring expenses; implements cost-saving actions.

    • Updates job knowledge by participating in educational opportunities; reads professional publications; participates in CTI-T programming; maintains personal networks; participates in professional organizations.

    • Enhances department and organization reputation by accepting ownership for accomplishing new and different requests; explores opportunities to add value to job accomplishment.

    • Client intake, documentation, or related experience requiring data collection, confidentiality, and attention to detail.

    • Ability to work on multiple projects, set priorities, and meet deadlines.

    • Strong time management skills; ability to manage downtime effectively to enhance productivity.

    • Excellent written and verbal communication skills; proactive in communicating task or project status to colleagues and management.

    • Demonstrated administrative acumen; general understanding of health provider operations requiring a keen attention to detail, task completion, and accountability.

    • Demonstrated client/customer service skills.

    • Proven conflict management and de-escalation abilities.

    • Ability to interrupt/challenge bias and discrimination and engage with people in a manner that promotes equity and inclusion.

    • Proficiency in the use of Microsoft applications and Google Business Suite.

    • Spanish-language proficiency is a plus.

    • Strong commitment to serving marginalized and disconnected communities.

  • The physical demands and work environment characteristics described here are representative of those that must be met by an employee to successfully perform the essential functions of this position.     • Physical demands: Sitting, walking, lifting up to 20 pounds, reaching, carrying, speaking, listening.

    • Work environment: An environment of high stress and fast pace of crisis intervention. The protocol for safety and security is a priority. Use of phones, in-person assistance, computer, fax, and other general office machinery.
